Everyone has a policy. Almost nobody applies it
The policy is written on the website and in the confirmation email. What happens next is where clinics lose the money and the goodwill at the same time.
Nobody wants the conversation
Your front desk knows there is a fee. Bringing it up with a patient standing at the desk, on a busy Saturday, is a different matter.

Set the two fees
A fee for a missed appointment, and a fee for a cancellation inside your notice period. Either can be nothing if you only want to use one of them.
Write the wording once

Mark the visit as usual
When your team marks an appointment missed, or a patient cancels late through their manage link, the fee is recorded on that visit automatically.
Waive it in one tap
A patient whose car broke down is not the same as a patient who has missed three. Your team clears the fee with one tap, and the record shows it was waived.
Keep a deposit against it
Where a deposit was already paid, it can be kept against the fee simply by not refunding it. Anything beyond that is settled at the desk.

Recorded, not charged. Read that twice
Some systems say they "automatically charge no-shows". That means they keep your patients' card details on file and take money from them without anyone at your clinic being involved.
Velvet Lead deliberately does not keep patients' card details. So the fee is recorded against the visit, a deposit already paid can be kept against it, and anything more is settled at your clinic however you already take payment. That is the honest version, and in our experience it is also the reassuring one: no patient has ever been pleased to discover a clinic can charge their card at will.
Shown before they book, not after they miss
A fee only works if the patient agreed to it while they still had a choice. The wording you write is shown during booking, in the same flow where they pick their time and enter their details.
That single change removes most of the awkward conversations. When a patient misses an appointment, your team is referring back to something the patient read, rather than introducing a rule at the worst possible moment.

The deposit does the heavy lifting
For the treatments where a missed appointment really hurts, a booking deposit is the strongest tool you have. The patient pays it on Stripe's secure page when they book, through your own Stripe account.
If they then miss the appointment, the deposit is simply not refunded, and the fee it covers is recorded against the visit. No card is charged after the fact, because the money already moved when the patient chose to commit.
Fees are the last line, not the first
A fee is what is left after everything else has failed. Most missed appointments are forgotten ones, which is why the practical work of reducing no-shows happens earlier: reminders by email and WhatsApp, a self-cancel link in every confirmation, and unconfirmed requests that expire on their own.
Pair fees with the waiting list and a late cancellation stops being pure loss. The patient cancels through their link, the fee is recorded, the slot is released, and the people waiting for that exact treatment hear about it within minutes.
What you can see afterwards
Every recorded fee sits on the visit it came from, next to the booking's status and history, so your team can see what happened and when. Missed appointments and late cancellations are already counted in reports by day, treatment and practitioner.
Before holding a valuable time, your front desk can also check a patient's own record for completed visits, missed ones and late cancellations over the past year. It is written as a note, never a judgement: people have hard years, and the decision about what to do with that stays with your team.
Honest limits
What fees don't do
This is the part most software is vague about, so here it is plainly.
- The fee is recorded, not collected. No card is kept on file and nothing is taken automatically.
- A deposit already paid can be kept against a fee. Everything beyond that is settled at your clinic.
- There is no invoicing, no point of sale and no full online payment of a treatment bill.
- Refunds are made in your own Stripe account, not from the Velvet Lead admin panel.
- Nothing here is legal advice. Your cancellation terms are yours to write and to check locally.

Questions clinics ask
Can we charge for missed appointments?
You can record a fee, and settle it at your clinic. Velvet Lead records the fee automatically the moment a visit is marked missed, and a deposit already paid can be kept against it. It does not keep patients' card details, so nothing is charged automatically.
Why not charge the card automatically?
Because that means keeping your patients' card details on file so money can leave their account with nobody at your clinic involved. We would rather not hold that, and most patients would rather we didn't. A deposit taken at booking achieves the same thing with the patient's agreement.
When exactly is a fee recorded?
The moment your team marks a visit as missed, or the moment a patient cancels inside your notice period through their own manage link. Nobody has to remember to add it afterwards.
Can we let someone off?
Yes, in one tap, and the record shows the fee was waived rather than never existing. That matters when the same patient's history comes up again later.
Do patients know about the fee before they book?
Yes. You write the wording yourself and it is shown to the patient during booking, before they confirm. A fee a patient never saw coming is an argument waiting to happen.
What happens to a course session when someone misses an appointment?
